BSA Medical Form (fillable)

• Parts A and B (pages 2 and 3 of the medical form file linked above) are completed annually by participants in all scouting events. Parts A and B of the medical form must be on file with the Troop or Crew for attendance at all of the troop's monthly campouts, Crew campouts and events. 
• Part C (page 4 of the medical form file linked above) is also completed for participants in any event that exceeds 72 consecutive hours, and for all high-adventure base participants. Part C (in addition to Parts A and B) of the medical form must be on file with the troop for attendance at summer camp and crew's high adventure activities.
• Part D (pages 5 through 14 of the medical form file linked above) is also completed for all high-adventure base participants. Part D is filed in addition to Parts A, B, and C of the medical form.
