Scout Physical
For camps up to 72 hours, only fill out pages 2-3 (Part A & B).
For longer camps, fill out pages 2-4 (Parts A, B, & C).  Part C requires a visit to a physician.
This is required annually for all scouts and leaders.

Tour Plan (Replaces Trip Permit)
This must be filled out and submitted at least 3 weeks before event.  Only needed for any of the following events:
          Camp outside of council
          Camps over 72 hours in length and outside of council
          High Adventure camps
          Camps over 500 miles away (except council camps such as Teton High Adventure).
